Complete Intake Form

Before your appointment, please print and complete our New Patient Intake Form, which you can find here. This form helps us gather essential information about your medical history and current concerns. Bringing it with you will save time during check-in.

 

But don't worry if you don't complete it in advance, we have copies at our clinic as well!

Arrive Early

To ensure that your appointment starts on time and you have ample opportunity to ask questions, we recommend arriving at least 15 minutes before your scheduled time. This will also give you a chance to complete any additional paperwork if necessary.

List of Medications

Please bring a list of all medications, supplements, and vitamins you are currently taking, including their dosages. This information is important for your treatment plan and overall health assessment. Your pharmacist can provide you with a printed list.

Remove Nail Polish

If you have nail polish on your toes, we kindly request that you remove it before your appointment. This will allow our chiropodist to perform a thorough examination of your nails and assess any potential issues accurately. Please note that if nail polish needs to be removed by our team, there may be an additional charge for this service.

Insurance Coverage

It’s important to be aware of your insurance coverage before your appointment. Please contact your insurance provider to confirm your coverage for chiropody services or orthotics. If you have any questions or need assistance, our friendly staff is here to help.

Referral

A referral is not needed to see a chiropodist; however, if your doctor has referred you to our practice, please bring the referral paperwork with you. This helps us understand your medical history and the specific reasons for your visit.

Diagnostic Reports

If you have recently undergone any diagnostic tests related to your foot or ankle condition, such as X-rays or ultrasounds, please bring the reports. These records are valuable for accurate diagnosis and treatment planning.

Footwear and Orthotics

For a comprehensive assessment of your foot health, please bring the footwear you most commonly wear, including everyday shoes, work shoes, athletic shoes, and any special-purpose footwear. This allows us to evaluate how your footwear may be contributing to or affecting your condition. If you currently use custom orthotics, please bring them to your appointment so we can assess their condition and effectiveness
